Yes, you can actually do that. It's not too terribly difficult either.

1. Choose a blank save and pick the space stage. Fly around near your homesystem and write down/memorize the arm location and surrounding solar systems. Then quit without saving.

2. Load another save at the space stage, one with at least Interstellar Drive 4 and a few terraforming and planet editor tools. Fly to the location in the correct arm and look for any familiar solar systems until you find the right one.

3. Terraform it up and save, go to the galaxy screen, and begin on your newly-refurbished homeworld!

And that's all you need to do to change the surface. Changing trees, however, is more risky. You will have to terraform the planet to T0 and then back to T3, which may cause glitches. You can put random creatures down, because it's not going to matter in the creature stage.

You can do this to the surrounding planets and moons, too. It's even possible to Planetbust them, although I definitely don't recommend doing that.

Would that make it possible to have one single continent or perhaps a chain of islands close enough to avoid the sea monster?

Epicalness. I shall go try it now.
As a test, of course.

Are you serious!?

*Facepalm* Lol. I always assumed it wasn't possible to teraform game planets. I'm so trying this out! (When my game works again, though).

P.S.

Hi. I'm a teenaged South African / Swede-to-be. A few things about me: I have a science obsession. I write and worldbuild in the realm of speculative fiction. I am highly liberal. I am an Agnostic Panentheist. I am a bi-curious homosexual. (Any problem with the last three is your own, thank you very much.) I am probably still alive. Left this place a long time ago. I can now be found here and here. I am now known as "Valdin", "TemporalV01D" and "SPACE_LEMON" because "DudeMan95" is a turdly username. DudeMan95 also had bad spelling and an occasional douchy attitude. My apologies. That is all. Enjoy the rest of your existence.

If you blow up that moon with the spaceship, you won't be able to advance past the beginning of the space stage. If you actually blew up the homeplanet, the game will probably crash when you attempt to play on that save.

I'm sorry but I still don't get how you know that you will get that planet when starting a new game. Do you make sure to type in that planet name? Or can you give it any name that you want and still get that one planet? Really, how do you know other than waiting till after naming it and hope you show up on it? I'm just curious....

Answer Time!!

1 - You can connect up the continents, but this will result in glitchyness during civ (only 4 cities appear, but each capture results in a quarter of the entire progress bar being filled up, so you'll still be avle to finish)

2 - You find the location by starting a space stage gem (as stated in OP), hover over the little spaceship icon, note down the numbers that apear (angle, distance...), quit WITHOUT saving, load up another empire, fly over there, scult and colour to taste, go back to main menu, start new game.

Have I missed anything out?

The optimist sees a glass as half full, the pessimist sees the glass as half empty and The Grox sees the glass as a threat and blasts it to pieces.

Call me slowwwwwww.

LOL I think I get what you are saying now.

By flying in, noting everything and quitting without saving.... But I am still wondering... When they have you name your planet, if changing the name changes the starting planet?

No, it's the same planet in the same location.
